Each photo requires a release form signed by the photographer and all identifiable subjects. Photographers must complete the form in full, including the email addresses of any visibly identifiable subjects. Photo subjects will receive an email to complete their release form.

Release forms will be sent to the email(s) registered on the fields above when you submit your form, and you will receive a copy once they have completed their form.

NOTE TO ENTRANT: Photos entered in the Jostens Photo Contest (“Contest”) must be accompanied by this Release Form, signed by all individuals who are personally and substantially identifiable within the photo (including crowds and large groups). Release Form may be printed out in multiple copies as needed and signed, then uploaded with your photo submission to www.jostens.com/photocontest (“Contest Website”). You may type in the information, print out and then have the individuals sign. Typed signatures will not be accepted. See Official Rules available on Contest Website for details.

This Release Form (“Release”) pertains to images from photographs in which the individual(s) whose signature(s) is/ are below (“I” or “Photo Subject”) appears, which photographs were taken by Entrant and will be entered by Entrant into the Contest, and any material based upon or derived therefrom (collectively, the “Images”). For good and valuable consideration received, the receipt and sufficiency of which is hereby acknowledged, I agree as follows: I hereby grant to Jostens, Inc., and its subsidiaries, and their respective employees, agents and others working for such entities on their behalf and their respective licensees, successors and assigns (collectively, “Jostens”) the absolute right and permission to upload, use, re-use, publish, broadcast and/or copyright the Images, in whole or in part, individually or in conjunction with any other images, in any medium or form including without limitation webbased media such as websites, posters, banner ads, email ads and other forms of media, throughout the universe in perpetuity, for any purpose whatsoever including without limitation those related to the sale, publication, display or exhibition thereof in education, promotion, advertising and any other trade or commercial purpose. I agree that Jostens shall not be liable for any distortion, alteration, optical illusion or use in composite form that may occur or result from Jostens’ use of the Images. I further agree that Jostens shall not be liable for the use, misuse, misappropriation or unauthorized use of the Images by any third party. I shall have no right of approval and no claim (including without limitation claims based upon invasion of privacy, defamation or right of publicity) arising out of Jostens’ use, alteration, distortion or illusionary effect or use in any composite form of the Images. I represent to Jostens that this Release does not in any way conflict with any existing commitment or obligation to which I am currently bound. I acknowledge that I am not entitled to receive any subsequent compensation from Jostens pursuant to this release.

If the subject is under 18 years old, the legal guardian must be the one to sign the form. Please forward the form to the legal guardian for completion. Click on the 'Save for Later' button and forward this form.

If Photo Subject Is Under 18 Years of Age: I represent and warrant that I am the parent or legal guardian of the above-referenced Photo Subject and that I agree to all of the terms and conditions contained in this Release Form.
